Bug/fixes for Sponsor Enrollment part 1

Hoping this is an effective way to show the screenshot for each example, and a description.

When selecting the state, it seems more logical to list it without the word "state" or to include the state along with all the programs they support.

The prefixes lack correct punctuation. This becomes problematic later because we attempt to handle them in a hard-coded manner, which isn't always suitable. The prefix "Honorable" should not have a period after it. This applies to "Mr." and "Mrs." but not to "Miss."

This shows a phone number already exists, meaning the same number cannot be used for multiple sponsors. Is that a true limitation?

These suggested addresses are not in the standard US order. Shouldn't this be standard US style? Though I'm not sure where the County goes in that case.

Here's another example of where the prefix punctuation ends up wrong.

The previous screen completes the first phase of enrollment. I think it's unnecessary to click the finish button before sending the email. The email should be sent before moving to the screen below. Clicking "Finish" should just return you to the main page.
